When Do You Need This Form?
After the Employee Card is issued, the employer must confirm that the foreigner has actually started working.

Company name, IČO, address.
Employee details
Field 2
Full name, date of birth, nationality, Employee Card number.
Start date
Field 3
The date the foreigner actually started working.

- This is the EMPLOYER's responsibility — make sure they know about it
- Failure to submit can cause problems with future renewals
- Ask your employer for a copy for your records
