Application for a long-term visa (D-type visa) allowing stay in the Czech Republic for more than 90 days. This is the first step for most third-country nationals arriving for study, work, or family reasons.

When Do You Need This Form?

When applying for a long-term visa (D visa) for stay over 90 days. Must be applied for from your home country at a Czech embassy/consulate.

  • Valid passport (at least 2 blank pages, valid 1+ year beyond planned stay)

  • Proof of purpose of stay
  • Proof of accommodation in CZ

  • Clean criminal record
  • Health insurance (comprehensive, valid in CZ)

  • All documents must be apostilled and translated to Czech
