Landlord details
Full name, birth number (rodné číslo) or date of birth, permanent address of the landlord.
Property address
Full address of the accommodation being provided.
Duration
Whether accommodation is for a specific period or unlimited.
Signature verification
The landlord's signature must be officially verified (úředně ověřený podpis).

- This is one of the most critical documents — without it, your application will be rejected
- The signature MUST be officially verified for long-term stays
- For short-term visa only, a hotel confirmation may suffice
- There are two versions: one for individual landlords (FO) and one for companies (PO)
- If your landlord is reluctant, explain it's a standard legal requirement
- Czech-only form — help your landlord fill it out if needed
