Date and place of birth

Your current nationality. If you hold dual citizenship, list the one matching the passport you used for your Czech visa.
Passport / travel document number
Number from the passport you used to enter / reside in the Czech Republic.
Current address in the Czech Republic
Your registered address (not mailing address). Must match the address on your residence permit.
Reason for requesting the rodné číslo
Briefly explain why you need it — e.g., "employer requirement", "bank account", "health insurance registration". Typically needed when it was not assigned with your residence permit.
