When Do You Need This Form?
When you want to see what documents and information OAMP has in your case file. Especially useful if your application is delayed or if you received a negative decision and want to understand why.
Fields people get wrong
Case reference number (Číslo jednací)
The reference number from any OAMP correspondence about your case. Format: OAM-XXXXX/DP-YYYY. Without this, they cannot locate your file.
Type of application / case
Specify which application the file relates to — e.g., long-term residence, permanent residence, Employee Card, etc.
Preferred date and time for inspection
Suggest when you would like to visit. OAMP will confirm or propose an alternative. Weekday morning is usually best.
Reason for inspection (optional)
You do not have to state a reason — it is your legal right. But noting "preparation for appeal" or "status check" can help expedite scheduling.
Personal details and contact
Full name, date of birth, address, phone/email. Must match the person named in the case file.
