When Do You Need This Form?
When OAMP has asked you to provide additional documents by a certain deadline and you need more time.
Fields people get wrong
Case reference number (Číslo jednací)
The reference number from the OAMP letter that set the deadline. Format is usually OAM-XXXXX/DP-YYYY. Found on the top of the official correspondence.
Original deadline date
The date OAMP gave you to submit the missing documents. Copy exactly from their letter.
Requested new deadline
Propose a specific new date. Be realistic — 2-4 weeks is typical. Requesting months may be denied.
Reason for extension (Důvod)
Explain clearly why you need more time: document still being processed, apostille not ready, translation not completed, waiting for foreign authority, etc. Be specific.
Personal details and contact
Your full name, date of birth, address, and phone/email so OAMP can notify you of the decision.
