The main application form for obtaining or renewing an Employee Card (zaměstnanecká karta). This is the most common work permit for non-EU foreigners working in the Czech Republic.

New application: Czech embassy/consulate in your home country, or at the regional MoI office (OAMP) if you are already legally in CZ. Renewal: Regional MoI office (OAMP) at least 120 days before expiry.

When Do You Need This Form?
When applying for a new Employee Card or renewing an existing one. Required for all third-country nationals who want to work in the Czech Republic under an employment contract.

Tips & Best Practices
- Start the process early — processing times can extend beyond 60 days
- Make sure the vacancy number is still active before submitting
- All documents must be originals or officially verified copies
- Foreign documents must be apostilled/superlegalized and translated to Czech by a sworn translator
- Keep a copy of everything you submit
- You can track your application status online at frs.gov.cz
