When Do You Need This Form?
When you want to change your employer, change your job position, or take an additional job at a different position or with another employer. Must be submitted at least 30 days BEFORE the change.

- Submit at least 30 days before the planned job change — this is a LEGAL REQUIREMENT
- Do NOT start working at the new position before MoI confirms or the 30-day period passes
- If MoI does not respond within 30 days, you can start — silence = approval

Common Mistakes to Avoid
- Starting the new job before the 30-day waiting period
