When Do You Need This Form?
When your residence card as a family member of an EU citizen is approaching expiry and you want to continue living in CZ.
Required Attachments
- Valid passport
- Current residence card
- Proof the EU family member is still in CZ
- Proof of continuing family relationship
- Proof of accommodation
Fields people get wrong
Personal details (Osobní údaje)
Full name, date of birth, nationality exactly as on your current residence card. Include maiden name if applicable.
Current residence card number and expiry
The number on your current card and its expiry date. Found on the front of your biometric residence card.
EU family member details
Full name, nationality, and Czech address of the EU citizen you are related to. They must still be residing in CZ.
Relationship to EU citizen
Specify: spouse, registered partner, parent, child, dependent relative, etc. Must match original application.
Address in CZ (Adresa pobytu)
Your current registered address. If you moved since the last card, provide the new address and proof of accommodation.
Employment / financial situation
Current employment status and employer details if working. This helps demonstrate you are not a burden on the social system.
Tips & Best Practices
- Apply at least 120 days before your card expires
- Your stay is legal while the application is pending (even if the card expires)
- Consider applying for permanent residence instead if you've been in CZ for 5+ years
