When Do You Need This Form?

After 5 years of continuous residence in CZ (or 2 years if you are a family member of a permanent resident EU citizen). Also available for humanitarian reasons or if born in CZ.

Tips & Best Practices

Continuous stay means you were not absent for more than 6 months at a time (or 12 months total in 5 years)
Permanent residence has no expiry — only the card needs renewal every 10 years
With permanent residence, you have nearly the same rights as Czech citizens (except voting)
