When Do You Need This Form?
When a non-EU national who is a family member of an EU citizen wants to stay in the Czech Republic for more than 3 months.
Required Attachments
- Valid passport
- 2 passport photos
- Proof of family relationship (marriage certificate, birth certificate)
- Proof that the EU family member is registered in CZ
- Proof of accommodation
- Proof of health insurance

- This gives you the RIGHT to work without any work permit

- The residence card is valid for 5 years (or the expected stay of the EU family member)
- After 5 years, you can apply for permanent residence
